The Opportunity:

The Lead Nurse Field Supervisor is a key clinical leadership role within the Patient Services team, responsible for overseeing both the Field Nurse Case Manger (FNCM) and the Nurse Care Navigator (NCN) teams. This role ensures consistent, high-quality delivery of patient support services offered by Sentrex Health Solutions.

The Lead Nurse Field Supervisor is responsible for coaching and mentoring staff, maintaining high standards of clinical practice, and ensuring full compliance with program protocols. The role also plays a critical part in fostering high-quality customer engagement by supporting strong professional relationships with clinics, physicians and client stakeholders. In addition, the Lead Nurse Field Supervisor serves as a key escalation point for complex cases and collaborates closely with cross-functional teams to optimize patient care, streamline workflows, enhance service delivery, and uphold client expectations. 

This is a remote based position, that will require time spent travelling to and working with clinics on an as needed basis. 

A Day in the Life (What you will do here):

  • Adhere to Sentrex Policies and Procedures.
  • Provide supervision, direction, and mentorship to both FNCMs and NCNs.
  • Conduct in clinic coaching and observation sessions to evaluate performance, strengthen clinical competencies, and ensure adherence to program best practices.
  • Serve as the primary escalation point for clinical questions, complex patient cases, and operational challenges. 
  • Support the onboarding and training of new FNCMs and NCNs.
  • Facilitate team meetings, case reviews, and continuous learning sessions.
  • Ensure all FNCM and NCN activities comply with Sentrex policies, program protocols, and applicable clinical standards.
  • Oversee adverse event reporting and ensure timely follow-through in accordance with pharmacovigilance requirements. 
  • Participate in internal quality audits and support corrective and preventative action initiatives.
  • Maintain consistent service delivery across field-based and virtual teams.
  • Collaborate with Program Manager, Associate Program Manager, Care Navigators, Clinic Solutions, and internal partners to ensure seamless care coordination. 
  • Monitor service metrics and regional trends for both FNCM and NCN teams, recommending operational improvements when needed.
  • Support territory balancing, resource allocation and coverage planning.
  • Troubleshoot operational barriers related to clinics, patient access and system workflows. 
  • Attend select clinic visits to support high-priority or high-complexity sites. 
  • Provide individualized coaching based on observations, performance trends, and case reviews. 
  • Support professional growth through structured feedback and development planning. 
  • Communicate program updates, new clinical information and best practices to the team. 
  • Build and maintain strong relationships with physician offices, clinics, and client stakeholders.
  • Represent the FNCM and NCN teams in cross-functional discussions and client meetings as needed. 
  • Support program improvements, pilot initiatives, and client-driven requests. 
  • Prepare reports, performance summaries and continuous improvement initiatives.
  • Prepare for and attend extensive, specialized training sessions for specific therapies provided by the pharmaceutical team.
  • Attend key patient and stakeholder meetings, as well as relevant conferences, to support FNCMs and NCNs in delivering high quality education on disease, product information, and case management services.
  • Liaise with physicians’ offices, pharmacies, infusion services providers and other parties to provide .comprehensive and coordinated service to patients and physicians and resolve issues as they arise.
  • Participate in ongoing internal and/or external continuing education activities.
  • Additional duties as requested by the Manager.

What you need to ensure you are set up for success:

  • Must be a Registered Nurse in good standing with the College of Nurses of their respective province.
  • 4+ years of clinical experience.
  • Experience in immunology, rheumatology, or dermatology an asset.
  • Strong analytical and problem-solving skills to address barriers and optimize patient care processes.
  • Ability to manage both field-based and virtual staff in a dynamic environment.
  • Demonstrates strong customer service skills, with the ability to build trust and rapport with HCPs, patients, and stakeholders.
  • Highly collaborative with effective and practical communication skills.
  • Experience in Patient Support Program is a strong asset.
  • Proven ability to work in a team environment.
  • Bilingual in English and French is an asset.
