The Business Planning Office (BPO):

The Business Planning Office (BPO), within the Performance and Business Intelligence portfolio at Trillium Health Partners (THP), serves as the enterprise project office for Reducing Hospitalization transformation project, one of THP’s most critical strategic priorities. The BPO leads the coordinated planning and execution of system‑wide initiatives that reduce preventable admissions, shorten length of stay, and enable earlier transitions home through seamless, connected care. Working in close partnership with the Senior Leadership Team, Patient Care Services, and enabling teams, the BPO integrates planning, performance, and project management practices across clinical programs, corporate portfolios, and external partners to translate strategy into measurable, enterprise‑level impact.
 

Position Summary:

The BPO is seeking a Senior Project Manager to join the team, with a primary focus on supporting THP’s Reducing Hospitalization transformation project.

Reporting to the Manager, Business Planning Office, the Senior Project Manager will lead and coordinate complex, multi-year, and cross‑portfolio initiatives that advance Reducing Hospitalization project objectives, working in close partnership with clinical leadership, corporate teams, and external stakeholders. The role functions as a central point of integration across planning, performance measurement, and execution, ensuring alignment between strategic priorities and project delivery.

 Key Responsibilities:

The successful candidate will help deliver on the core priorities of the Business Planning Office, with specific emphasis on the Reducing Hospitalization transformation project, including:

  • In partnership with Patient Care Services, Institute for Better Health, Quality, Performance Analytics, Business Intelligence, AI & Digital Planning, Finance, Human Resources, and other enabling teams, support the planning, coordination and monitoring of complex, multi-year projects that span multiple portfolios, to enable the delivery of Reducing Hospitalization initiatives across the organization
  • Lead the development, implementation, and ongoing refinement of governance, project management tools and standards, and reporting structures for work related to Reducing Hospitalization.
  • Support integration across Reducing Hospitalization initiatives, including alignment between planning cycles, performance management processes, and project execution
  • Translate strategic objectives from THP’s Plan to 2030 into clear project plans, milestones, deliverables, and performance measures, ensuring transparency and accountability for progress
  • Partner with clinical, enabling and executive leaders to identify system‑level opportunities to reduce avoidable hospitalization and improve patient flow and care transitions
  • Support the identification and alignment of short, medium, and long‑term provincial and regional health priorities with THP’s Reducing Hospitalization transformation project
  • Prepare clear, concise, and executive‑ready materials, including briefing notes, presentations, and reports for senior leadership and governance forums
  • Provide mentorship, guidance, and day‑to‑day support to BPO staff and project team members to enable individual and team success

Qualifications:

  • Undergraduate degree in business, life sciences, health sciences, health informatics, or a related discipline required
  • Master’s degree in public administration, business administration, health administration, or equivalent experience required
  • Project Management Professional (PMP) designation considered a strong asset
  • Experience within consulting or complex health system transformation initiatives preferred
  • Minimum of 4+ years of progressive project management experience in healthcare environments such as hospitals, government, or private sector
  • Demonstrated experience managing complex, multi‑year initiatives with the ability to define and track clear goals, milestones, risks, and key performance indicators
  • Experience implementing and translating performance dashboards and analytics into actionable insights
  • Demonstrated understanding of hospital operations and health system dynamics
  • Proven ability to anticipate the needs of senior leaders and support executive‑level decision‑making
  • Strong analytical, critical thinking, and problem‑solving skills
  • Demonstrated effectiveness working with large, complex, and multidisciplinary teams
  • Excellent interpersonal, communication, facilitation, and stakeholder engagement skills
  • Demonstrated ability to develop high‑quality reports and presentations for executive, administrative, and clinical audiences
  • Strong organizational, time management, and prioritization skills
  • Proficiency in Excel, PowerPoint, and Microsoft Office Suite
  • Experience with Epic and digital health initiatives considered an asset
  • Satisfactory performance and attendance required
